Information about you will not be passed on by a health care professional without your written consent. Please inform us if there is anything in your medical history which you would not want anyone to see who is not responsible for your care.
Medical Records
You can apply to see your own medical records, but you can also apply:
On behalf of your child, with your child’s consent, if your doctor decides it is in the best interest of your child.
On behalf of someone who is not capable of giving his or her permission (you would have to go through a court to be able to do this).
To see the records of someone who has died if you are acting as their personal representative.
You can also see your medical records on the NHS app, simply download the app on your device.
All staff are fully trained in confidentiality and sign a disclaimer, please be reassured that all of your information is strictly confidential, your appointments are confidential and no information will be shared without your permission.
